Port forwarding not working..!!

I've tried to port forward.. but it does not show my router model.. Is there any other alternative? I have tried the first time.. my ip works.. and the second it still works.. the third time and after that my ip wont work again..  :'( HELP ME!!!

What is the name of ur Router and Which company of router and ur COuntry name Define here?
