It can happen when the other player, in your game, blow up in the car, but he isn't blowed up, he has leaved the car before the explosion. The reason of this bug is the lag.

Is easy, when you exit from a vc mp server use /q. If this problem repeats you must open the task manager and you must close the vc mp process from process list.

Forze that is a bug (i mean the second), they are spawned but you are bugged and you can't see them. That bug is frequently in USM DM. I don't know the first bug.
